Abe is a 33-year-old father of five children who has been diagnosed with cardiomyopathy. His disease is in its early stages with a few bothersome symptoms. Abe is overweight and has a sedentary job as a bookkeeper. He also has hypertension and orthopnea. The case manager enters Abe's room and tells him that he must be discharged by the end of the day.
