CLIENT HISTORY Age: 75 VITAL SIGNS:
SYNOPSIS Gender: Female Blood pressure: 116/70
  Height: 4'11" Pulse rate: 72
CASE B Weight: 105 lbs Respiratory rate: 15

MEDICAL HISTORY:
The client reports psychiatric treatment for depression and Parkinson's disease
SOCIAL HISTORY:
The client is a high school graduate, homemaker, and widowed 5 years ago. Since her only income is Social Security, family members provide assistance and transportation.
MEDICATIONS:
The client is taking Paxil® (20 mg qid) and Larodopa® (two .5 g tabs bid).
CHIEF COMPLAINTS:
"Some of my teeth look bad and my gums bleed, so I want my teeth cleaned and my cavities fixed."
DENTAL HISTORY:
The client has had extensive restorative treatment in the past, but has not seen a dentist for nearly 5 years. The oral hygiene is poor with heavy plaque and materia alba accumulations throughout the mouth.
ADDITIONAL FINDINGS:
Facial structures are somewhat rigid with a mask-like appearance. Manual dexterity is limited, verbal responses are slow and brief and muscle tremors occur periodically.
SUPPLEMENTAL DENTAL FINDINGS:
Bleeding on probing.
