An 84-year-old woman has recently returned home after being admitted to the hospital for persistent N/V and dehydration. Her past medical history includes gastric reflux, hiatal hernia, GI bleed, anemia, and CAD. Her current medication regimen is: Lopressor 50mg PO q a.m., Lopressor 25mg PO q p.m., Protonix 40mg PO daily, Lasix 20mg PO daily, Colase 100mg PO BID, Remeron 15mg PO at h.s., and Zofran 4mg PO before meals.
